Job Summary and Responsibilities
nThis PRN position provides coverage for East Valley.
nWe are seeking a skilled and compassionate Speech Language Pathologist to join the Dignity Health at Home team. The Speech Therapist will assess, diagnose and treat patients with speech, language, communication and swallowing disorders for patients in their homes. This role involves developing individualized treatment plans tailored to each patient's needs and working collaboratively with families and other healthcare professionals to achieve optimal patient outcomes.
nResponsibilities:
nnnProvide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.

nnCustomize therapy strategies to meet individual patient goals and objectives.
n
nnMaintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
n
nnProvide training and support for family members to assist with therapeutic exercise and strategies at home.
n
nnDignity Health Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time.
n
n nJob Requirements
nQualifications :
nnnMasters degree in Speech-Language Pathology from an accredited program.
n
nnActive registered SLP license in the state which you practice and certification (CCC-SLP) from ASHA.
n
nnMust obtain and maintain current CPR/BLS Certification
n
nnOne-year Speech Therapy experience in a clinical setting.
n
nnCurrent Drivers License
n
nnComply with federal, state and local laws and regulations
