Job description

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

nSpeech Language Pathologist

nPRN Position

nRange: $50 - $60 per hour

nServicing Grand Rapids and the surrounding areas

nnnDirect and administer speech therapy using techniques that assist in the rehabilitation of individuals with functional disabilities of speech, language, voice, and swallowing.

nnReview medical records, test and evaluate individuals served, develop treatment plans, and administer therapy.

n

nnProvide effective evaluations, documentation, and communication of medical information.

n

nnAssess speech and language skills, development, articulation ability, and voice and fluency disorders of patients assigned.

n

nnDevelop, implement, and evaluate individualized patient therapy plans, coordinating therapy modifications to the individual's plan of care with the Director of Clinical Services and the individual's physician.

n

nnIdentify and develop intervention strategies; routinely review pertinent medical data to determine the effectiveness of therapy services in reaching maximum rehabilitation potential for each individual served.

n

nnInstruct individuals served, family members, and primary caregivers as needed to ensure the maintenance of or acquisition of optimal functioning level for each individual.

n

nnEnsure effective coordination of home care services through timely completion of required documentation and transfer of pertinent medical data to the individual's physician, Director of Clinical Services, and other caregivers.

n

nnReview and document comprehensive evaluation reports to summarize tests utilized, results of test(s), and diagnosis of speech/hearing disorders.

n

nnReport and record assessments, evaluations, therapy interventions, and patient changes according to physician orders and the Mentor Network's policies.

n

nnInvestigate and take appropriate actions on all concerns and/or complaints.

n

nnMaintain appropriate HIPPA compliance and a safe patient environment.

n

n nQualifications:

nnnMaster's Degree in Speech Therapy or Speech/Language Pathology.

n

nnOne year's experience as a Speech Therapist.

n

nnCurrent state licensure/certification as a Speech Therapist.

n

nnSelf-motivated and detail-oriented with ability to multi-task.

n

nnExcellent communication and analytical skills.

n

nnCommitment to a multidisciplinary team approach.
