nJob Summary
nWe are seeking an experienced, licensed Speech Therapist to join our team! As a Speech Therapist, you will be meeting with new patients, assessing their speech needs, and creating personalized treatment plans to help them reach their goals. You will also be documenting all patient care, observing and participating in treatment, and adjusting goals and milestones as needed for the best possible outcomes.
nResponsibilities
nProvide therapy services to patients in their homes
nMake evaluations and create treatment plans customized for that patient, working with them to set short and long term goals for their speech and wellness
nAssist in the implementation of the treatment plan, adjusting as needed to ensure successful outcomes
nMaintaining excellent records of patient needs, care, and outcome
nWork closely with the family of the patients and the patients itself to ensure they have the knowledge and tools they need for success
nQualifications
nActive NY state license for practicing speech therapy
nStrong communication and interpersonal skills
nExcellent organizational skills
nStrong charting and documentation skills
nPrior experience in home care is preferred but not required
