Speech Language Pathologist (SLP) - Home Health | Per Diem

We are currently seeking an experienced Speech Language Pathologist (SLP) to join our growing team on a per diem basis supporting adult and geriatric clients throughout Hudson County, NJ.
nThis is an excellent opportunity for an SLP looking for flexibility, autonomy, and meaningful one-on-one patient care.

n nWhat You'll Do
nAs a Home Health Speech Language Pathologist, you will:
nnnProvide in-home speech therapy evaluations and treatments for adult and geriatric clients
n
nnAssess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders
n
nnDevelop individualized treatment plans in collaboration with physicians and care teams
n
nnModify care plans as patient needs evolve
n
nnEducate patients and caregivers to promote safety, independence, and improved quality of life
n
nnCollaborate closely with nurses, therapists, and interdisciplinary team members
n
nnComplete timely electronic documentation using a tablet-based EMR system
n
n nQualifications
nnnCurrent NJ Speech Language Pathologist license required
n
nnCertificate of Clinical Competence (CCC-SLP) preferred
n
nnHome health experience preferred but not required
n
nnStrong communication and organizational skills
n
nnComfortable with electronic documentation/EMR systems
n

nBase Pay, depending upon qualifications and subject to negotiation:
n$65.00 - $75.00 per point
