Job Overview

nCrossview Care Center is seeking a compassionate and dedicated PRN  Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. We are hiring PRN Speech-Language Pathologists to provide high-quality therapy services in our skilled nursing and subacute rehabilitation setting.

nAs a Speech-Language Pathologist, you will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. You will collaborate with an interdisciplinary team to develop individualized treatment plans that promote functional recovery, safety, and improved quality of life following illness, injury, surgery, or hospitalization.

nEssential Duties and Responsibilities

nnnPerform comprehensive evaluations of speech, language, cognitive-communication, and swallowing function

n

nnDevelop and implement individualized treatment plans based on physician orders, clinical findings, and resident goals

n

nnProvide evidence-based interventions for speech, language, cognition, voice, fluency, and dysphagia disorders

n

nnAssess swallowing function and recommend appropriate diet textures, strategies, and safety precautions

n

nnMonitor resident progress and modify treatment plans to maximize functional outcomes

n

nnComplete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines

n

nnCollaborate with physicians, nursing staff, physical therapists, occupational therapists, dietitians, and other interdisciplinary team members to coordinate resident care

n

nnEducate residents, families, and caregivers on communication strategies, swallowing safety, cognitive rehabilitation techniques, and home exercise programs

n

nnParticipate in interdisciplinary care conferences, family meetings, and discharge planning

n

nnMaintain compliance with all professional, regulatory, and facility policies and standards

n

nnSupport a resident-centered rehabilitation environment focused on quality outcomes and clinical excellence

n

n nQualifications

nRequired

nnnMaster's degree in Speech-Language Pathology from an accredited program

n

nnCurrent Georgia Speech-Language Pathologist license in good standing

n

nnCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; Clinical Fellowship (CF) candidates may be considered

n

nnCurrent CPR certification or willingness to obtain upon hire

n

nnStrong clinical assessment, communication, and documentation skills

n

nnAbility to work effectively within an interdisciplinary healthcare team

n

n nPreferred

nnnExperience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation

n

nnExperience with dysphagia management and cognitive-communication disorders

n

nnFamiliarity with Medicare documentation and reimbursement guidelines

n

nnExperience working with geriatric and medically complex populations

n

n
