Job description
Sweetwater Care is seeking a Speech-Language Pathologist to assist patients with speech, language, swallowing, and cognitive disorders in a SNF setting. You will conduct assessments, develop treatment plans, and document progress while collaborating with physicians, therapists, and caregivers.
Responsibilities include evaluating patients, planning therapy, and ensuring high-quality rehab services in alignment with physician orders and Medicare/Medicaid guidelines.
