Job description
CenterWell Home Health seeks a compassionate Speech Language Pathologist to provide in-home assessment and treatment for adults with communication and swallowing disorders. In this field-based role, you will perform comprehensive evaluations, develop individualized care plans, and deliver evidence-based interventions that promote safety, independence, and quality of life. You’ll collaborate closely with an interdisciplinary team, educate patients and caregivers, and maintain accurate EHR documentation.
