Speech-Language Pathologist (SLP) – Home Health

First Choice Home Health & Hospice is hiring a Speech-Language Pathologist (SLP) to provide skilled home health therapy on a part-time or PRN basis.

This is a great opportunity for an SLP who values flexibility, professional autonomy, and meaningful one-on-one patient care.

You'll evaluate and treat patients in their homes while collaborating with nurses, therapists, physicians, and other members of the interdisciplinary care team.

Qualifications

Required:

Current Utah Speech-Language Pathologist license in good standing

Graduate degree in Speech-Language Pathology

Strong clinical assessment and communication skills

Ability to work independently in patients' homes

Valid driver's license

Reliable transportation

Preferred:

Home health experience

Adult or geriatric therapy experience

Experience with dysphagia

Experience with cognitive-communication disorders

Experience treating neurological conditions or post-stroke patients

Responsibilities

Evaluate speech, language, cognitive, and swallowing function

Develop individualized treatment plans

Provide skilled speech therapy in patients' homes

Treat communication, cognitive, speech, language, and swallowing disorders

Educate patients, families, and caregivers

Monitor progress and adjust treatment plans as needed

Coordinate care with physicians and the interdisciplinary team

Complete accurate and timely clinical documentation

Communicate changes in patient condition or care needs

Schedule

Part-Time / PRN

Patient visits are scheduled based on patient needs, with flexibility to organize your schedule throughout the week.

Compensation

$80–$105 per completed visit, based on visit type.

Work Setting

Patient homes and residential care settings within the assigned service area.
