Job Description
Speech-Language Pathologist - Home Health
Schedule
Self-scheduled visits, with a requirement of 6-8 home skilled visits per day. Availability for 1-2 weekends per contract.
Assignment Details
Work requires travel across multiple territories. Contract lengths vary based on the needs of the office.
Requirements
1 year of home health experience required
Valid driver's license and personal vehicle for home visits
Certification in BLS and CCC-SLP
State Licensure required
OASIS training preferred
COVID vaccination or exemption form on file
Flu shot or declination documented, with mandatory mask wearing while at work
Responsibilities Conduct evaluations using various screening tools such as BIMS, depression screening, and oral peripheral exams
Develop and implement dysphasia management and aural rehabilitation plans
Educate patients and families regarding therapy plans and patient safety
Maintain documentation and billing in compliance with regulations
Provide in-service training as required
Implement functional maintenance programs for patients
