nMedical Social Worker (MSW) PRN

nElara Caring is hiring a compassionate PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.
nThis position gives you the independence to work directly with patients in their homes while remaining connected to an experienced interdisciplinary team. Your guidance, advocacy, and community knowledge can make a lasting difference for patients and their families.

n nWhat You’ll Do
nnnAssess patients’ psychosocial, emotional, financial, and environmental needs
n
nnComplete initial social work evaluations and follow-up visits
n
nnProvide counseling, education, advocacy, and emotional support
n
nnConnect patients and caregivers with community programs and financial resources
n
nnIdentify barriers affecting patient safety, recovery, or independence
n
nnHelp patients and families navigate healthcare and community services
n
nnParticipate in developing and updating individualized Plans of Care
n
nnCollaborate with physicians, nurses, therapists, and other care team members
n
nnSupport discharge planning and continuity of care
n
nnCommunicate concerns or changes affecting patient well-being
n
nnComplete accurate and timely clinical documentation
n
n nWhat You’ll Bring
nnnMaster’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
n
nnCurrent Maine Social Worker license in good standing
n
nnAt least one year of social work experience in a healthcare setting
n
nnStrong counseling, communication, documentation, and advocacy skills
n
nnKnowledge of community programs and available patient resources
n
nnAbility to work independently in patients’ homes
n
nnWillingness to travel throughout the assigned service area with 50% travel required
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n
nnMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
n

nThe base salary for this position is $70.00 to $80.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
