Job description
We are seeking a Home Health Speech Language Pathologist (SLP) to provide personalized speech therapy services in patients’ homes, focusing on communication, swallowing, and cognitive impairments resulting from illness or injury.
Key responsibilities include:
Conducting assessments, documenting progress, and ensuring patient goals are met
Implementing therapy exercises for speech, swallowing, and cognition
Educating patients and families on care plans, safety, and self-care practices
Collaborating with a multidisciplinary home health team to deliver comprehensive care
Qualifications and skills required:
Master’s or Doctoral degree in Speech Language Pathology and active license in Colorado
At least one year of experience in speech therapy, preferably in home healthcare
CPR certification and ability to pass background and drug tests
Knowledge of home health regulations, strong clinical judgment, empathy, and excellent communication skills
