Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities in Long Island, NY.
We are looking for compassionate and dependable clinicians who are passionate about providing high-quality, one-on-one care while helping patients improve communication, speech, language, cognition, and swallowing skills in the comfort of their homes.
Responsibilities
Evaluate patients and develop individualized treatment plans
Provide skilled speech-language therapy based on each patient’s needs and goals
Monitor patient progress and adjust treatment plans as appropriate
Complete accurate and timely documentation
Communicate effectively with office staff, patients, and caregivers
Collaborate with the clinical team to ensure coordinated and effective treatment
Work with healthcare professionals, including speech therapy assistants, nurses, physicians, and other members of the care team, to support comprehensive patient care
