Speech Language Pathologist (SLP) - Phoenix, Arizona
At Desert Peak Care Center, we are dedicated to delivering exceptional patient care and creating a supportive work environment for our team members. If you are a qualified medical professional with a passion for occupational therapy, we invite you to join our team as a Speech Language Pathologist (SLP).
As an SLP, you will evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. You will work closely with an interdisciplinary team to help residents regain their independence and improve their quality of life.
Key responsibilities include:
Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing disorders.
Provide direct therapy services in accordance with the plan of care.
Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive resident care.
Educate residents, families, and staff on communication strategies and safe swallowing techniques.
Document all evaluations, treatments, and resident progress in compliance with state, federal, and facility regulations.
Participate in care planning meetings, discharge planning, and quality improvement activities as needed.
Provide training and supervision to Speech-Language Pathology Assistants (if applicable).
Qualifications include:
Master's degree in Speech-Language Pathology from an accredited program.
Current Arizona licensure as a Speech-Language Pathologist (or eligibility to obtain).
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
Current CPR certification (or willingness to obtain).
Experience in a skilled nursing facility or with geriatric populations preferred.
