Job description
Territory: Santa Cruz County — Watsonville
All opportunities at 21HHS require being in the field visiting patients in their homes.
What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):
Provide skilled services to assist patients with breathing, speaking, and swallowing functions in partnership with the Case Manager
Evaluate and treat speech, language, cognitive and fluency disorders that affect communication
Serve as an educator, patient advocate, and a member of the interdisciplinary care team
