Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards.
Educate residents, families, and staff on communication strategies, compensatory techniques, and safe swallowing.
Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.
Qualifications
Current, unrestricted Speech-Language Pathologist license in the state of Minnesota (or eligibility prior to start).
CCC-SLP preferred; strong CFs with relevant LTC/acute rehab experience may be considered based on client need.
Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred.
Excellent communication, documentation, and collaboration skills; resident-centered, outcomes-focused approach.
