nPHS Rehabilitation is seeking a Part Time experienced Speech Language Pathologist, CCC-SLP certification required, to serve in our assisted living and independent living in Waukesha, Pewaukee, Oconomowoc, and Muskego area.

n nResponsibilities
nAs a Speech Language Pathologist you will be responsible to:
nnnConduct a comprehensive assessment & evaluation using subjective and objective measures.
n
nnDevelop treatment plans and programs in accordance with physician orders.
n
nnProvide quality treatments, actively progressing goals to achieve highest quality of life.
n
nnConduct screens and provide education to residents/clients, families, caregivers, and staff.
n
nnCoordinate seamless discharges utilizing the full continuum of therapy and fitness services.
n
nnProvide accurate and timely documentation & billing in electronic medical record.
n
nnDemonstrate understanding and knowledge of payer sources and regulatory requirements.
n
nnComplete and document supervisory visits per regulations.
n
nnCommunicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
n
n nQualifications
nnnGraduate from an accredited Speech Language Pathology program.
n
nnCurrent licensure in the state(s) of practice as required.
n
nnDemonstrated compatibility with PHS's mission and operating philosophies.
n
nnDemonstrated ability to read, write, speak and understand the English language to communicate with all customers.
