Schedule: Hospital pharmacy coverage; specific shift details to be confirmed

Requirements Active Kentucky Pharmacist license required
Previous hospital or LTAC pharmacy experience required
Current BLS certification required
EPIC experience preferred
Experience with discharge medication reconciliation
Strong patient and family counseling skills
Knowledge of antimicrobial stewardship and culture result interpretation
Experience with pharmacokinetic dosing and monitoring
Anticoagulation dosing and monitoring experience
Strong communication and clinical decision-making skills
