Job description
HCR Home Care in Onondaga County, NY is seeking a full-time RN Start of Care Nurse for CHHA duties. The role includes patient assessments, care coordination, and adherence to NYS guidelines.
The RN will perform initial assessments, manage the Start of Care admission process, complete documentation, and collaborate with physicians and case managers to deliver high-quality home care.
