Job description
TEEMA Solutions Group is seeking a Director of Case Management for a full-time role in Bradenton, FL. The director will oversee hospital case management operations, ensuring quality care coordination and discharge planning.
The ideal candidate should possess strong change management skills and have a minimum of 3 years' experience in acute care. A bachelor's degree is required, and a master's is preferred. RN or LCSW licensure is also a must.
