Job description
Mount Sinai Health System is seeking a Director of Pharmacy – Transitions of Care (TOC) to provide strategic and operational leadership for enterprise-wide TOC programs. You will develop, implement, and optimize TOC initiatives across inpatient, ambulatory, and community settings to improve medication safety and reduce readmissions.
The role requires PharmD or advanced degree, PGY1, and preferably PGY2 in related fields, with 7–10 years of leadership experience and BCPS/BCACP certification.
