Job description
The Manager, Social Impact & Population Health at Hackensack Meridian Health will lead and manage the team of Community Health Workers and the Program Coordinator to deliver community-based support to high-risk, low-income patients to help them reach their health goals.
This is a grant-funded position requiring a Master’s in social work or public health, at least three years of leadership experience, and strong knowledge of NJ healthcare and public benefit enrollment.
