Job description
Healthcare in Action delivers street medicine to unhoused individuals using mobile clinics in California, working with SCAN and other partners. The Primary Care NP leads clinical care within a team that includes social workers and peer navigators, delivering direct care in field settings.
You will perform exams, create care plans, conduct chronic assessments, coordinate with physicians, educate patients, and ensure proper documentation, referrals, and care coordination.
