Street Medicine Program Medical AssistantReports to Office ManagerCahaba Medical Care Foundation is a community health center organization providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, and Chilton counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission accredited organization, committed to increasing integration and coordination of behavioral health and primary care. This is an exciting, fast paced practice with a strong mission and commitment to providing high quality care.Purpose: To assist providers and other clinical staff in delivering efficient, high-quality patient care as part of the Cahaba Street Medicine Program. The Medical Assistant will provide clinical, phlebotomy, administrative, and patient support services in community-based and mobile settings while helping connect patients with ongoing medical care and community resources.Travel to various community outreach locations and assigned sites is required to complete job responsibilities.Principal Responsibilities:Assist providers and clinical staff with patient care in mobile, community, and outreach settingsTriage patients and obtain and document vital signsObtain and document patient medical history, medications, allergies, and other pertinent informationDocument all patient care and services in the electronic medical record (EMR)Prepare patients for examinations, procedures, and treatmentsAssist providers with: Performing examinations, procedures, and minor surgeries Ordering standard laboratory tests for chronic disease visits Completing quality management activities in the EMR Reconciling medications and allergies E-prescribing medications after appropriate changes have been made by the providerPerform phlebotomy, including blood draws, specimen collection, labeling, processing, and preparation for transportPerform waived laboratory testing as ordered by the provider upon satisfactory assessment of competencyAssist with patient testing and screenings, including other diagnostic services upon satisfactory assessment of competencyAdminister medications and vaccines upon satisfactory assessment of competencyAssist with wound care and remove sutures or staples from superficial wounds, as well as casts, splints, and other external devices, as directed by the provider and upon satisfactory assessment of competencySet up, organize, and break down mobile clinical areas and equipmentEnsure medical supplies, medications, equipment, and clinical materials are available and properly stored for outreach activitiesClean and stock clinical areas and mobile units and maintain accurate inventory recordsRespond to patient messages and assist with scheduling appointments, procedures, referrals, imaging, and follow-up careAssist patients with navigating appointments and connecting with appropriate community resources and servicesSupport patient outreach and engagement efforts to promote continuity of careComplete patient paperwork, including prior authorizations, referrals, home health paperwork, medical supply paperwork, and other required documentationWork through assigned tasks in the EMR and maintain accurate and timely documentationCommunicate with physician offices, pharmacies, laboratories, hospitals, shelters, community organizations, patients, and other ancillary healthcare providers as directed by the providerProvide administrative support as needed, including answering phones, scanning and filing documents, maintaining patient records, and coordinating patient informationMaintain patient confidentiality and follow all applicable HIPAA, safety, infection control, and organizational policiesDemonstrate flexibility and professionalism while working in a variety of community-based environments and with diverse patient populationsQualifications:Satisfactory completion of CMCF Medical Assistant Training ProgramCertified Medical Assistant preferred but not requiredPhlebotomy experience or certification preferredPrevious experience in a medical office, community health, outreach, or underserved population setting preferredExcellent customer service and communication skillsStrong organizational skills and attention to detailBasic computer and electronic medical record experienceAbility to manage time, multitask, and work effectively in a fast-paced environmentAbility to work independently and as part of a multidisciplinary teamAbility to adapt to changing environments and community-based work settingsCompassionate and respectful approach to working with individuals from diverse backgrounds and circumstancesValid driver's license and reliable transportation, as required for assigned travel
