OPS Program AssistantThe Streetlight Program Assistant herein supports the Streetlight program's mission to provide age-appropriate peer support to chronically ill adolescents as a model of pediatric palliative care. The Streetlight Program Assistant assists with administrative duties, patient support, and volunteer support.Administrative SupportRecruits, screens, and orients new volunteersReviews EPIC charts and filters patient census and medical information into a Streetlight team-member appropriate "need-to-know" formatRecords daily patient referrals from medical staff and consolidates information into task-lists for team membersManages Streetlight encrypted SharePoint site featuring weekly team educational readings, announcements, and educational web linksManages REDCap database including transcription of patient demographics, diagnoses, and volunteer psychosocial chart entriesFacilitates communication and collaboration with interdisciplinary team members to best meet the needs of patients enrolled in StreetlightVolunteer SupportAssists with new volunteer orientation and continued education in palliative care, chronic and terminal illness, HIPAA compliance and best practices to provide psychosocial care to the target populationConducts project-based volunteer meetingsOrganizes and archives team education materialsCoordinates team member graduation ceremony and other social eventsPatient SupportIntroduces Streetlight program to new designated patientsProvides psychosocial support to patients including inpatient room visits, outpatient appointment visits, and event planningProvides end-of-life support including creation of legacy projectsRegularly coordinates activities for patients/families (milestone events, unique long-term patient events, etc.) including management of logistics, catering, and community involvementOrganizes weekly meal deliveries for Cystic Fibrosis patients through the BJ's CF Dining Days ProgramMaintains Streetlight active patient database, used for program censusExpected Salary: $20 per hourRequired Qualifications: Qualifications refer to evidence-based or demonstrated knowledge, skills, abilities, and experiences required for the role. For TEAMS positions the required qualifications should reflect Classification Minimum Requirements.
