nConcern Housing is seeking a Substance Use Case Manager who will be responsible for providing case management and chemical dependency counseling services directly to clients receiving Mobile Residential Support Team services. The Substance Use Case Manager is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access and also be an advocate for their clients.
nDays/Hours:Monday - Friday; 9am - 5pm
nResponsibilities Include but are not limited to:
nnnProvision of chemical dependency counseling services to assist individuals to develop the skills needed to prevent relapse, promote recovery and independence and increase self-sufficiency. Counseling shall include assessment of needs, identifying triggers, increasing coping skills, development of resources, service planning, referral to necessary treatment providers, case coordination and collaboration, community integration, supportive counseling and crisis intervention.

nnProvision of case management services (assistance in forming linkages and/or referrals to clinical services, day programs, medication management, life skills counseling services vocational services, and opportunities, entitlements etc.)

nnTimely submission of all documentation, and maintenance of all client charts in compliance with agency standards.

nnAttendance at supervision and staff meetings.
n
nnOther duties as assigned by the Director of Supported Housing.

nnOverall assessment of client welfare and appropriate intervention and follow-up as needed.

nnRemain current with all Agency required compliance documents and trainings.

n nRequirements:
nCASAC credential is required for this position. Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related discipline, plus 2 years’ experience working in a similar setting and/or working with people in recovery from chemical dependence. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
