The online report will be accepted as long it is printed from the web browser, by clicking the 'print' button displayed on the Driver History Report page (The Driver History Report page cannot be accepted, you MUST click the print button and save as a PDF to attach); copy/paste, word, rich text, etc., versions will not be accepted. You must attach the DMV Printout as a PDF.
