Job description
Position Summary
nThis position is responsible for management and administration of day to day operations of the community based services program including: management and support of case management services; management of record keeping and data quality, supervision of assigned staff and interns, and compliance with all agency and funder requirements.
nEssential Duties and Responsibilities
n Case Management Oversight and Supervision
nnAssist in landlord mediation when issues arise between tenants and landlords
nAssist case managers with challenging cases, accompanying staff to home visits as needed
nOversee the Community Based Services staffing schedule and ensure coverage during absences
nProvide crisis intervention in mental health and/or medical emergencies
nWork with case managers to ensure goal achievement in the areas of housing, income and health
nMonitor program outcomes and ensure continuous quality improvement
nEngage in and provide clinical supervision in both group and individual formats
nWork with tenants to address issues in order to prevent eviction
nManage and distribute petty cash, transit cards, and other resources for case managers
nWith other Community Based Housing Lead Case Manager, host team meetings to ensure communication and staff development opportunities for community-based case managers
nCreate systems and documents to improve compliance and time management for staff
nn n Record Keeping and Data Quality
nnRegularly review files to ensure timeliness and accuracy
nWork collaboratively with the Program Data Manager to ensure data quality
nComply with HUD guidelines and participate in monitoring/site visits with HUD, the CoC, the Center for Housing and Health and other parties as need
n nnQualifications and Requirements
nEducation: Bachelor's Degree in Social Work or related field. Master degree preferred.
nLicensure and/or Certification: Professional Licensure in the State of Illinois (LSW, LCSW, LCPC) or equivalent preferred.
nnJob Experience: Minimum 2 years of case management or social service experience. Minimum 2 years of supervisory experience.
nnSpecial Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, effective oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and own transportation required.
