Summary:
POSITION OVERVIEW
This position plays a vital role in providing in-home supportive services with a holistic approach and provide seamless care to the tribal elders and patients of the Community Health Nursing Division. Providing services such as medical transports, assisting with grocery shopping, running errands, light meal preparation, and light housekeeping duties if medically indicated on their plan of care. While also providing emotional support and companionship to patients and respite services for family members.
MINIMUM QUALIFICATIONS
EDUCATION:
1. Must have a high school diploma or equivalent.
ESSENTIAL:
1. Must be 18 years of age or older.
2. Valid driver's license, dependable transportation and proper car insurance are required.
3. Must promote a healthy, non-abusive lifestyle.
4. Must be able to pass a pre-employment background check and will be subjected to periodic checks throughout employment.
5. Current First Aid/CPR certification or must be obtained within one (1) month of hire date.
6. Complete National Incident management systems (NIMS)/Incident Command Systems (ICS) 100 and 700 certification within six (6) months of hire date.
7. Personal Care Worker Certificate must be completed within six (6) months of hire date.
