GLIDE is seeking a Transitional Aged Youth Health & Wellness Case Manager to Outreach, assess, counsel, and coordinate care for TAY ages 18-27 across housing, health, education, and employment domains.
Responsibilities include intake, goal planning, care coordination with partners, and leading psychoeducation groups. Strong communication and independence are essential for success in this role.
