Responsibilities
As a Hospice Team Coordinator, You will:
Provide administrative support to clinical leadership and care teams.
Maintain organized patient records, office files, and supplies.
Answer phones, manage mail, and assist visitors with professionalism.
Track and order medical equipment and office supplies.
Input data into electronic systems (e.g., HCHB) and set up clinician tablets.
Assist with scheduling, admissions, discharge, and team coordination.
Support HR functions like maintaining employee records.
Ensure compliance with hospice policies and Medicare regulations.
Qualifications
High School Diploma or GED required.
Minimum of two years of administrative healthcare work experience, preferably in hospice and/or home health operations.
Ability to maintain orderly files, supplies, and office systems.
Proficient in verbal and written communication with patients, families, and staff.
Familiarity with data entry systems such as HCHB and basic office software.
Understanding of Medicare documentation standards and referral/intake
