Job description
Affinity Home Health Care Inc is seeking a Family Nurse Practitioner to deliver high-quality care in clinical and virtual settings. You will diagnose, prescribe, and educate across all ages, while leading clinic staff to uphold clinical standards.
You’ll document patient data, interpret results, order tests, and manage acute and chronic conditions. This role includes onboarding, mentoring staff, and some on-call duties, with a strong emphasis on telehealth and team collaboration.
