Key Responsibilities
Conduct comprehensive mental health assessments that consider the patient's physical health and co-occurring conditions, ensuring an individualized treatment approach.
Develop, implement, and evaluate integrated treatment plans that holistically address both psychiatric and medical issues, guiding patients through the complexities of their health.
Provide individual and group therapy sessions, using evidence-based methodologies tailored to the specific needs and circumstances of each patient.
Collaborate with healthcare providers, ensuring coordinated care through regular communication and case reviews to enhance treatment outcomes.
Offer immediate crisis intervention to support patients experiencing mental health emergencies, employing de-escalation techniques and safety planning.
Educate patients about the interplay between mental and physical health, thereby empowering them to take active roles in managing their overall wellness.
Maintain meticulous records and documentation in compliance with HIPAA regulations and ethical guidelines, ensuring patient confidentiality.
Engage with team members during case conferences and meetings to contribute to patient-centered care planning and decision-making.
Stay informed of the latest research, best practices, and advancements in integrated mental health care to continuously enhance service delivery.
Patient Population / Client Focus
The ideal candidate will work with a diverse patient population, including individuals with complex mental health needs exacerbated by physical health conditions. You will provide holistic support that addresses not just their psychological wellness but also their medical circumstances, facilitating a comprehensive treatment journey.
Qualifications
Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
Active and valid licensure as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT in New York State.
Strong understanding and experience in treating individuals with co-occurring medical and mental health conditions.
Exemplary interpersonal and communication skills, facilitating effective relationships with patients and colleagues alike.
Ability to uphold professional boundaries, demonstrating ethical practice and confidentiality in all aspects of care.
Proficiency in electronic health record systems and telehealth technologies to streamline patient care and documentation.
Preferred Experience
Prior experience in a telehealth or remote service delivery environment.
Experience working within an integrated care model, collaborating with interdisciplinary teams.
Familiarity with crisis intervention strategies and trauma-informed care approaches.
Compensation and Benefits
