Pay & Benefits:
Hourly pay rate: $63.65

What You'll Do:
Evaluate patients' physical therapy needs across a diverse age range, from pediatric to geriatric.
Develop and implement individualized treatment plans to improve mobility and independence.
Communicate effectively with patients, families, physicians, and healthcare team members.
Document patient care activities accurately and timely in electronic medical records.
Assist with patient transfers and positioning to ensure comfort and safety.
Monitor and adjust treatment plans based on patient progress and goals.
Collaborate with healthcare teams to optimize patient rehabilitation outcomes.
What You Need:
Valid Illinois licensure (or willingness to obtain)
CPR certification
At least 1 year of relevant PT experience
Strong communication and patient interaction skills
Experience working in home health or outpatient settings
Bilingual abilities or additional languages
