Job Summary:
Provide specialized occupational therapy services to patients in a skilled nursing facility, focusing on promoting independence and improving functional abilities. Collaborate with care teams to develop and implement effective treatment plans tailored to individual patient needs.
Responsibilities:
Assess patients' functional status and develop personalized therapy plans.
Implement therapeutic interventions to improve motor, cognitive, and sensory functions.
Document patient progress and adjust treatments accordingly.
Educate patients and families on therapy goals and home exercises.
Collaborate with healthcare team members to ensure seamless patient care.
Adhere to regulatory standards and facility protocols.
Qualifications: Current occupational therapy license in the appropriate state.
Demonstrated experience in a skilled nursing or similar setting preferred.
Strong knowledge of therapeutic techniques and patient care standards.
Excellent communication and interpersonal skills.
Ability to develop individualized treatment plans based on patient assessments.
Certifications in CPR and basic life support.
