Job Summary:
Seeking a licensed Physical Therapist to provide comprehensive rehabilitative services in a skilled nursing facility. The therapist will evaluate patients, develop treatment plans, and deliver personalized care to support recovery and mobility goals.
Responsibilities:
Assess patient conditions and establish individualized treatment plans.
Deliver therapeutic interventions to improve mobility, strength, and functionality.
Collaborate with nursing and healthcare teams to coordinate care.
Document patient progress and adjust treatments as needed.
Educate patients and families on therapeutic exercises and care plans.
Qualifications: Hold a current and valid Physical Therapist license.
Demonstrate experience in skilled nursing or long-term care settings.
Possess strong assessment and communication skills.
Be able to work a flexible schedule including weekend rotations.
Follow all applicable state and federal licensing requirements.
