Job Summary:
Provide comprehensive physical therapy services to patients in a skilled nursing facility setting. Assess, develop, and implement treatment plans to improve or restore patients' mobility and quality of life.
Responsibilities:
Evaluate patients' physical abilities and diagnose potential issues.
Develop individualized treatment plans based on patient assessments.
Implement therapy interventions and monitor patient progress.
Educate patients and their families on exercises and techniques to support recovery.
Collaborate with healthcare team members to coordinate care plans.
Document patient care activities and progress accurately.
Qualifications: Current valid state licensure as a Physical Therapist.
Proven experience working in a skilled nursing or similar setting.
Strong assessment and clinical reasoning skills.
Excellent communication and patient education abilities.
Familiarity with rehabilitation equipment and therapy techniques.
Ability to work independently and as part of a multidisciplinary team.
