Job Summary:
Provide skilled physical therapy services to patients in a home health setting, focusing on restoring mobility and functional independence. Promote healing and prevent further injury through personalized treatment plans.
Responsibilities:
Assess patient needs and develop individualized therapy programs.
Deliver physical therapy interventions during home visits, ensuring patient safety and treatment effectiveness.
Document patient progress and update treatment plans regularly.
Educate patients and caregivers on exercises, safety precautions, and ongoing care.
Maintain compliance with all clinical guidelines, policies, and state regulations.
Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes.
Qualifications: Hold a valid Physical Therapist license in the state of practice.
Minimum of 1 year of recent adult home health physical therapy experience.
Proficient in OASIS and EMR systems, specifically HomeCare HomeBase.
Excellent clinical assessment and communication skills.
Ability to travel up to 75 miles from the assigned clinic.
Reliable transportation and current driver's license with agency-required liability insurance.
