Job Summary:
Seeking a qualified Physical Therapist for an assisted living and memory care setting. The therapist will provide comprehensive rehabilitation services to enhance patient mobility and function.
Responsibilities:
Evaluate patient conditions and develop individualized treatment plans.
Administer therapeutic exercises and interventions to improve functional abilities.
Document patient progress accurately using electronic health records.
Collaborate with a small, experienced rehab team to ensure optimal patient outcomes.
Qualifications: Hold a valid state license as a Physical Therapist.
Proven experience in assisted living or memory care environments preferred.
Demonstrate strong assessment and treatment planning skills.
Proficient in electronic documentation systems such as Net Health.
