Job Summary:
Provides physical therapy assessments and interventions to a diverse patient population, supporting recovery and functional independence. Works under supervision within a multidisciplinary team across inpatient, outpatient, and acute care settings to deliver personalized care plans.
Responsibilities:
Conduct initial examinations to assess patient needs and establish treatment goals.
Develop, implement, and modify treatment plans based on patient progress and re-evaluations.
Document all clinical activities, including evaluations, progress notes, and discharge summaries in a timely manner.
Educate patients, families, and caregivers on treatment techniques, home exercises, and equipment use.
Collaborate with healthcare team members to coordinate discharge planning and ongoing therapy needs.
Participate in departmental meetings, conferences, and training sessions to enhance clinical practice.
Qualifications: Hold a current New York State Physical Therapy license.
Demonstrate excellent communication, interpersonal, and problem-solving skills.
Possess 0-1 year of clinical experience.
Be a member of the American Physical Therapy Association (APTA).
Bachelor's degree in Physical Therapy required; Master's degree preferred.
