n Seeking a dedicated Physical Therapist to provide comprehensive rehabilitation services in a skilled nursing facility setting. The role involves developing and implementing individualized treatment plans to improve patient mobility and function.n
nn Responsibilities:n
n n Evaluate patients’ physical conditions and establish personalized treatment goals.n
n Develop and modify therapeutic interventions based on patient progress.n
n Educate patients and their families on therapeutic exercises and wellness strategies.n
n Collaborate with healthcare team members to ensure cohesive patient care.n
n Document patient assessments, treatment plans, and progress notes accurately.n
n Follow all regulatory and safety standards related to patient care.n
nn Qualifications:n
n n Current and valid Physical Therapist license.n
n Proven experience working in a skilled nursing facility (SNF) setting preferred.n
n Strong knowledge of rehabilitation techniques and modalities.n
n Excellent communication and interpersonal skills.n
n Ability to assess patient needs and adapt treatment plans accordingly.n
