Job Summary:
Seeking a dedicated Speech Language Pathologist to assess, diagnose, and treat communication and swallowing disorders in a skilled nursing facility setting. The therapist will provide comprehensive care to patients across a skilled nursing unit on a full-time basis.
Responsibilities:
Evaluate patients' speech, language, and swallowing functions to determine appropriate treatment plans.
Develop and implement individualized therapy programs to improve communication skills and swallowing safety.
Collaborate with interdisciplinary teams to coordinate patient care and progress.
Document assessment findings, treatment goals, and progress notes accurately and timely.
Educate patients and families on therapeutic exercises and strategies to support ongoing communication enhancement.
Maintain compliance with regulatory standards and facility policies.
Qualifications: Valid state license or certification as a Speech Language Pathologist.
Proven experience working in a skilled nursing or long-term care setting.
Strong assessment and treatment planning skills.
Excellent communication and interpersonal abilities.
Ability to work effectively in a team environment.
