Job description
n Call for Salary Rate –n
nn Job Summary:n
n Provides evaluation, treatment, and rehabilitation services for patients with communication, cognitive, or swallowing disorders. Develops tailored care plans to improve patient outcomes while ensuring compliance with regulatory and clinical standards.n
nn Responsibilities:n
n n Assess patients’ communication, cognitive, and swallowing functions using standardized tools and evidence-based methods.n
n Design and implement individualized therapy plans based on thorough evaluations.n
n Monitor and document patient progress, adjusting treatments as necessary in accordance with clinical guidelines.n
n Communicate effectively with patients, families, caregivers, and multidisciplinary team members regarding treatment goals and progress.n
n Complete comprehensive discharge summaries and all required documentation within electronic health records.n
n Collaborate in team meetings and participate in ongoing training and facility initiatives.n
nn Qualifications:n
n n Hold a Massachusetts State License and possess active CCC certification as a Speech-Language Pathologist.n
n Have a minimum of 1 year of clinical SLP experience, preferably in LTC or rehab settings.n
n Demonstrate proficiency in evaluating and treating communication, cognitive, or swallowing disorders.n
n Be committed to adhering to ASHA standards, ethical practice, and regulatory requirements.n
n Ability to supervise and support CFYs or student SLPs.n
n Strong communication, documentation, and collaborative skills.n
