Job description
Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Hoover, AL. The Pharmacist will collect and assess clinical data, implement and monitor care plans, and dispense specialty medications while coordinating with prescribers to optimize patient outcomes.
The role requires a Pharm D or R.Ph with clinical experience, active licensure, and a willingness to work on-site with occasional on-call duties.
