Key Responsibilities
Provide occupational therapy treatment services to residents under the supervision of a licensed Occupational Therapist
Implement individualized treatment plans in a skilled nursing facility setting
Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
Collaborate with the interdisciplinary care team to support resident goals and outcomes
Required Qualifications
Current COTA licensure or certification required
Active state licensure or eligibility to practice in Wisconsin
Experience working in a skilled nursing or long-term care environment
Preferred Experience / Skills
Prior SNF or long-term care experience strongly preferred
Familiarity with Medicare documentation requirements
