Contract | 13-Week Assignment | Days | Monday-Friday, 8:00 AM-5:00 PM | Onsite
Key Responsibilities
Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward functional goals
Collaborate with interdisciplinary care team to coordinate patient care
Provide patient and family education related to therapy goals and home programs
Required Qualifications
Active Physical Therapist (PT) license in the state of Florida
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Preferred Experience / Skills
Experience in subacute rehabilitation settings
Experience in skilled nursing facility (SNF) environments
Background working with adult and geriatric patient populations
