Therapy - Physical Therapist (PT)
Shift Details: 07:00 AM - 03:00 PM, 5 shifts per week, 40 scheduled hours.
Job Order Details: Start date 10/26/2026, end date 01/25/2027, duration 13 weeks. No float or call required.
Client Details: Clinic setting in Griffin, GA, zip code 30223.
Cancellation Policy: Guaranteed hours. If a provider reports to work and begins their assignment but is no longer needed by Upstream, the client may "call off" the provider. In such cases, the agency may invoice Upstream for either the actual hours worked or up to four (4) hours of service—whichever is greater—at the standard weekday bill rate. I Cancellation: If your provider cancels less than 14 days prior to starting, a fee of 12 hours x the bill rate will be assessed. If your provider terminates without 30 days' notice, a fee of 36 hours x the bill rate will be assessed. These fees are passed directly to the facility per the contract terms.
