Key Responsibilities
Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established goals
Collaborate with interdisciplinary care team to support patient outcomes
Required Qualifications
Active Physical Therapist (PT) license in the state of Maryland
Graduate of an accredited Physical Therapy program
Prior experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills
Experience with geriatric patient populations
Familiarity with MDS documentation and Medicare guidelines in SNF environments
