Job description
Physical Therapist - Home Health
The Home Health Physical Therapist plans and administers medically prescribed physical therapy treatments for patients suffering from injuries, muscle, nerve, joint, and bone diseases. The goal is to restore function, relieve pain, and prevent disability. Duties include providing education and information to patients and caregivers. Participation in required discharge planning is necessary. All required documentation must be completed according to policy. Directing patient participation in selected tasks to restore, reinforce, and enhance performance is essential. Mileage will be reimbursed at the IRS rate. The unit is open to first time travelers. MatrixCare or Epic experience is preferred but not required. Candidates must review history thoroughly before submission. The facility requires a clear fit testing history; issues will eliminate candidates from consideration. This position may involve weekend coverage for orthopedic cases only. Candidates should have a compact license or be licensed in the state of Virginia. Documentation of any required time off must be provided for review and approval. Testing for rhythm identification and charting may be required before completion of orientation. Candidates must ensure they meet all compliance and testing requirements prior to starting.
