Physical Therapist
Job Title: Physical Therapist

License: CA PT License
Certifications: Home Health experience
Must-Have: Required to prioritize start of care visits.
Description: Required to supervise Physical Therapy Assistants. Next day mandatory reporting of start of care visits. Clinical notes and other documentation due within 48 hours of visit. Maintain timely communication with supervisor, intake, and office. Be generally responsive during normal business hours. Territory may expand as needed. Schedule is 40 hours per week, Monday-Friday, with one weekend a month rotation. Training involves 3 full days, one in-house and two in the field. Documentation is done using an electronic system. Equipment issued includes an iPad and a communication device. Mileage reimbursement is provided for travel between patients. Company car option is available. Productivity requirement is based on visit type and point value. Caseload and visit mixes per day will average 8 hours per day and 40 hours per week. Overtime must be pre-approved by the Clinical Supervisor.
