Job description
Home Health Physical Therapist
Join our team as a Home Health Physical Therapist, providing essential care to patients in their homes.
What You'll Be Doing
Deliver physical therapy services to adult and geriatric patients
Manage a caseload of approximately 7 visits per day
Utilize Home Care Home Base (HCHB) for patient documentation
Complete an online HCHB course if lacking experience
Adhere to compliance and orientation requirements
Work major holidays as required by assignment
What We Need To See
Active GA Physical Therapist License or Compact PT qualification
Current BLS certification
Minimum of 1 year of experience in physical therapy
Experience with Epic system
Ways To Stand Out From The Crowd
Experience in Home Health physical therapy
Specialization in geriatrics, strokes, and orthopedics
Familiarity with Home Care Home Base (HCHB)
